On or about June 27, 2013, in the production department, the electrical panel on the RF-14 sealing machine had energized wires running through the door which was closed with duct tape, exposing employees to an electrical hazard.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: a. On or about June 27, 2013, in the production department near the RF-11 sealing machine, the electrical conduit that was hanging from the ceiling did not have adequate strain relief.
