29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   a) On or about 07-03-2013 at the job site, where the roof (5/12 slope) was being decked the use of fall protection was not enforced, exposing the employees to a 30 ft. fall hazard.

29 CFR 1926.453(b)(2)(v): A body belt and/or harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:   a) On or about 007-03-2013 at the site, an employee working from a Genie S-60 4X4 aerial lift installing wood panels on the side of the building was not provided with a body belt/harness and lanyard and was exposed to a 18 ft. fall hazard.
