Pro Roofing & Associate, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 457016, Serious Citation number 02, item number 001, issued on July 12, 2012, with respect to a workplace located at 1100 Lime Street, Fernandina Beach, Florida, and was affirmed as a final order on July 31, 2012.    Pro Roofing & Associate, Inc. was previously cited for a violation of this occupational safety and health standard, or its equivalent standard, 29 CFR 1926(b)(11) which was contained in OSHA inspection number 311822530, Serious Citation number 01, item number 004, issued on August 6, 2009, with respect to a workplace located at 2971 Waller Street, Jacksonville, Florida, and was affirmed as a final order on August 14, 209.    29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:    a.   On or about July 5, 2013, on the 2:12 pitch roof, employees installing roofing materials were exposed to a 9 feet fall hazard in that they were not protected by a fall protection system.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards:  a.   On or about July 5, 2013, employees working from elevations had not been trained to recognize fall hazards, or the procedures to minimize those hazards.
