29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   On or about July 05, 2013, the employer did not ensure employees engaged in roofing activities were trained to recognize and avoid fall hazards and the procedures to be followed in order to minimize or avoid those hazards.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  On or about July 05, 2013, employees engaged in roofing work on a low sloped roof were exposed to a fall of over 10 feet to the ground below.   The Norberto Martinez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 312122286, citation number 1, Item 2 and was affirmed as a final order on October 15, 2009, with respect to a workplace located at 6055 LBJ Frwy, Dallas, TX 75118.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   On or about July 5, 2013, the employer did not ensure the ladder that the employees used to gain access to the standing seam roof extended at least 3 feet above the upper landing surface.  Employees were exposed to fall hazards.     The Norberto Martinez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 312122286, citation number 1, Item 3 and was affirmed as a final order on October 15, 2009, with respect to a workplace located at 6055 LBJ Frwy, Dallas, TX 75118.
