OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by unrestrained pipes handled by heavy equipment: 928 Gresham Road on jobsite, in front of warehouse. An employee was struck by unrestrained pipe that fell off a CAT 950E loader while standing between the forks of the loader and a flatbed trailer. On or about 07/05/13.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being crushed by heavy equipment that was overloaded: 928 Gresham Road on jobsite, in front of warehouse. A CAT 950E loader tipped forward while unloading pipe that exceeded its static tipping load capacity. On or about 07/05/13.
