29 CFR 1910.106(e)(9)(iii): "Waste and residue." Combustible waste material and residues in a building or unit operating area shall be kept to a minimum, stored in covered metal receptacles and disposed of daily.    In the Maintenance Shed, in the shop area, combustible rags were stored in an open bucket and were not disposed of daily, exposing employees in the area to a fire hazard and potential thermal burns.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    Employees handling and mixing golf course chemicals, including, but not limited to, Mec Amine-D, were subject to chemical burns to the eye when the employer did not provide a suitable eye wash station as defined in ANSI Z.358.1-2009 for flushing the eyes.

29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:     The employer did not block and stabilize the piping storage area located above the chemical storage room in the Maintenance Shed.  Piping was not secured and extended off the edge of the structure up to 12 inches maximum, exposing employees to struck-by injury if the piping were to roll off the structure.  A toe board would be one method to stabilize the stored piping.

29 CFR 1910.215(a)(2): Abrasive wheel(s) used on grinding machinery were not provided with safety guard(s) which covered the spindle end, nut, flange projections:    On the Reel Grinder or lawn mower sharpening machine, the abrasive wheel was not fitted with a safety guard covering the spindle end, nut and flange.  The wheel periphery shows where a guard was once attached; employees are exposed to injury from flying debris if the wheel were to break apart during use.

29 CFR 1910.219(e)(1)(i): Horizontal belts which had both runs 42 inches or less from the floor level were not fully enclosed by guards conforming to requirements specified in 29 CFR 1910.219(m) and (o):  In the Maintenance Shed, the reel grinder, or lawn mower sharpener, had an unguarded, horizontal flat belt drive (belt less than 1 inch wide) with the top belt located approximately 42 inches from the floor, and the bottom belt approximately 38 inches from the floor.  One end of the drive was adjacent to an unguarded grinding wheel; the belt drive created a caught-in hazard for employees.

The employer did maintain MSDS, labeling, and provided training to employees on the products they worked with.
