(a) Barlow Tyrie, located at 1263 glen Ave, Suite 230, Moorestown, NJ, Warehouse: Employees requiring access to the upper levels of a storage rack were being lifted on a pallet by a TCM forklift; model FCB15E3, instead of an approved platform, occurred on or about 7/8/13.
