29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:   On or about July 2, 2013, at the facility, the employer did not have an eye wash station for quick drenching or flushing of the eyes and body for employees handling corrosive chemicals and melting solid lead including but not limited to 15 % by weight, Molybdenum Disulfide OSHA TWA: 15 mg/m^3; 15 % Acetone OSHA TWA: 2,400 mg/m^3; and lead OSHA TWA: 0.050 mg/m^3. This condition exposed employees to dermal hazards while working with and around the chemical agents.   Instance a:  Employees were painting gun primer sleeves, which exposed employees to conjunctiva and dermal hazards while performing painting operations.   Instance b:  Employees were cutting and melting lead for ammunition projectiles, which exposed employees to dermal hazards associated with the accumulation of dust while performing the tasks.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:    On or about July 2, 2013, at the shop, where the employer did not ensure unused openings on the Galvanized Steel Square Electrical Conduit / Outlet Junction Boxes were closed or covered. This created an electrical shock, burn and fire hazard.

This condition exposed employees to inhalation ingestion hazards while working with and around lead.
