29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: a) On or about July 08, 2013, and at times prior, employees were exposed to a fall of approximately six feet to the lower roof and approximately 11 feet from the lower roof eave to the ground below while using a ladder to enter/exit the roof that was not set up in the manner it was designed. An eight foot step ladder was set on the roof without the ladder side rails extended as they were designed to be set.
