On or about July 08, 2013, at the above located workplace, employees traveled on the manlift (Humphrey Manlift, SN 2836) on a daily basis. The employer had not performed weekly checks on the manlift's limit switches.     Employees travelling on the manlift were thereby exposed to stuck-by and fall hazards.
