29 CFR 1910.68(e)(1): All manlifts shall be inspected by a competent designated person at intervals of not more than 30 days. Limit switches shall be checked weekly. Manlifts found to be unsafe shall not be operated until properly repaired. a. On or about July 08, 2013, at the above located workplace, employees traveled on the manlift (Humphrey Manlift, SN 2836) on a daily basis. The employer had not performed inspections by a competent designated person at intervals of 30 days or less. a. On or about July 08, 2013, at the above located workplace, employees traveled on the manlift (Humphrey Manlift, SN 2836) on a daily basis. The employer had not performed weekly checks on the manlift's limit switches. Employees travelling on the manlift were thereby exposed to stuck-by and fall hazards.

29 CFR 1910.68(c)(7)(ii)(a): At the top floor an illuminated sign shall be displayed bearing the following wording: "TOP FLOOR - GET OFF" Signs shall be in block letters not less than 2 inches in height. This sign shall be located within easy view of an ascending passenger and not more than 2 feet above the top terminal landing. a. On or about July 08, 2013, at the above located workplace, employees traveled on the manlift (Humphrey Manlift, SN 2836) on a daily basis. On the upper landing terminal, the sign that read "TOP FLOOR GET OFF" was not illuminated.

29 CFR 1910.68(c)(7)(ii)(b): A red warning light of not less than 40-watt rating shall be provided immediately below the upper landing terminal and so located as to shine in the passenger's face. a. On or about July 08, 2013, at the above located workplace, employees traveled on the manlift (Humphrey Manlift, SN 2836) on a daily basis. Immediately below the upper landing terminal a red warning light was not available to warn employees of the top floor.
