29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) West Point facility, buildings 12, 17, 28, 29, 38, 44, 45A, 45, 46, 60, 62, 66A, 66, 69, 71, 75A, 76, and 81- The employer did not enforce the use of developed written energy control procedures during maintenance and servicing activities on various makes, models, and sizes, of autoclaves, sterilizers, rack washers, and cage/cart washers to prevent the unexpected release of hazardous energy, when employees are assigned to enter the equipment to clean the device and remove debris build-up. Employees are exposed to potential serious injury caused by the release of this energy, with the hazardous conditions determined to exist on or about May 29, 2013.
