29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:   On or about 07/01/13 at Macon, GA: Employees were exposed to fall hazards, while performing masonry work on the 3rd buck of a framed supported scaffold without proper fall protection.  The employer failed to provide fall protection for employees working on a scaffold facing a large window opening on an exterior wall.

29 CFR 1926.452(c)(2): Tubular welded frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which would have secured vertical members together laterally:   On or about 07/01/13 at Macon, GA: Employees were exposed to fall hazards while performing masonry work on a 4 buck high framed supported scaffold. The employer failed to ensure that all scaffold frames were connected by the use of cross braces.    a. The 6th frame at the 4th buck high level was missing one cross brace pair on outside.  b. The 5th frame at the 1st and 2nd levels were missing both pairs of cross braces.

29 CFR 1926.452(c)(5)(iii): Brackets used to support cantilevered loads shall be used only to support personnel, unless the scaffold has been designed for other loads by a qualified engineer and built to withstand the tipping forces caused by those other loads being placed on the bracket-supported section of the scaffold:  On or about 07/01/13 at Macon, GA: Employees were exposed to fall hazards while performing masonry work on a framed supported scaffold. The employer placed masonry material (mortar) on a scaffold extension at the 4 buck high level.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement:  On or about 07/01/13 at Macon, GA: Employees were exposed to impalement hazards while performing work on a scaffold that was erected on the roof of a structure.  The employer failed to guard protruding reinforced steel that was located below the working level of the scaffold.

The employer failed to develop and implement a written hazard communication program.
