29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    On or about July 5, 2013, employees installing an HVAC units worked on the roof without fall protection exposing the employees to falls of more than 6 feet to the ground below.

29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  On or about July 5, 2013, employees were using an aluminum extension ladder with side rails that did not extend at least 3 feet above the edge of the roof.  The employees were exposed to a fall hazard.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed.    On or about July 5, 2013, employees were using an aluminum "A" Frame ladder that was leaning up against the wall to gain access to another section of the roof.  The ladder was not resting on all four legs exposing the employees to a fall hazard.
