29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:      On or about July 10, 2013, the following equipment had a path to ground which was not permanent, continuous and effective:    A.  At the Okuma Cadet #21 lathe, a 4-outlet pendant power drop, which provided power to a metal-encased inspection light and Daewoo microwave oven, was missing a ground.    B.  At the Okuma MX-45VBE Vertical Machining Center, a power was transmitted to the metal ring near the turn knob on a plastic-encased 2-wire magnifying lamp.  The lamp was not provided with a ground wire.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:      On or about July 10, 2013, at the Okuma Cadet #21 CNC Lathe, the flexible cordset on the Daewoo microwave oven was damaged, with the insulation pulled from the cord near the plug.  The cord had been wrapped with black electrical tape.  The metal parts of the equipment showed an open ground.
