29 CFR 1926.303(c)(5): Safety guards were not mounted as to maintain proper alignment with the wheel, and the guard and its fastenings were not of sufficient strength to retain fragments of the wheel in case of accidental breakage. The maximum angular exposure of the grinding wheel periphery and sides exceeded 180 deg: a) 126 N. Main Street, Manville, NJ 08835: Employees were using a Bosch 1380 Slim 4-1/2" Small Angle Grinder to cut metal framing without the safety guard installed over the blade. Condition observed on or about July 8, 2013.

29 CFR 1926.1053(b)(6): Ladders were used on surfaces which were not stable and level: a) 126 N. Main Street, Manville, NJ 08835: Employees were using utilizing a 20' Werner extension ladder that was placed on top of an unsecured piece of plywood located on top of an unfinished metal suspended ceiling to access the roof of the building. Condition observed on or about July 8, 2013.
