29 CFR 1910.119(d)(3)(i)(A): The employer did not complete a compilation of process safety information pertaining to the material of construction of equipment in the process.     (a) Process safety information regarding piping material of construction was not accurate. The process safety information material of construction for the piping does not accurately indicate the schedule of pipe to be used for 12 inch diameter pipe through 2 inch diameter pipe, potentially leading to wrong schedule of pipe being installed into process equipment.     Process Safety Information Section 4.2.8 Piping, identified that  "All pipe 12" diameter or smaller shall be schedule 80, SA 106, grade B, seamless." Section 4.2.8 further indicates that "All pipe 2" diameter through 10" diameter shall be Schedule 40, SA53, grade B, ERW, or SA106, grade B."

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment:     (a) Filler Machine - Use of Interlocks: The filler machine was not effectively rendered inoperative and locked out when performing "cleaning in place (CIP)" operations. The four control circuit interlocked access doors to the filler machine were opened and maintained in an open position during the CIP process. Mechanical processes and energy sources were not locked out, exposing employees to crushing, struck by hazards and caught in hazards.

29 CFR 1910.151(c):     Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:     (a) In the area of the ammonia refrigeration compressor/machine room, workers' eyes and bodies were potentially exposed to anhydrous ammonia, a corrosive chemical.  No suitable emergency eyewash and safety shower were installed just outside the area of the compressor/machine room's exit door (in accordance with IIAR Bulletin 109).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) Sharp Metal Lathe (Precision Machine Tools): Maintenance employees use the metal lathe to fabricate parts for food processing equipment and other application within the food processing operations. The lathe did not have a chuck guard in place, exposing maintenance employees to a caught on /caught in hazard.
