29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energiizng, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a) Shop Area - The employer failed to establish a program that included energy control procedures for employees to follow, employee training, and periodic inspections to protect employees performing set up and maintenance/servicing from injury caused by unexpected start up and energy releases.    In accordance with 29CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     (a) Building 170 S. Lively / CNC Area - The sliding barrier on Mori Seiki SL-65 CNC CL-4 machine would not prevent entry of hands into the point of operation and rotating parts during operation. The interlock device did not stop the machine from operating when the sliding barrier was opened.    (b) Building 170 S. Lively / CNC Area - The sliding barrier on Mori Seiki CL-3 machine would not prevent entry of hands into the point of operation and rotating parts.  The machine was not equipped with an interlock device to stop the machine from operating when the sliding barrier was opened.    (c) Building 200 S. Lively / Shop Area- The unused portion of the Grob vertical band saw (GS2) was not guarded.         In accordance with 29CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

(a) Shop Area - The Delta abrasive wheel grinder did not have an adjustable tongue guard.
