29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: a) 644 East 3rd Avenue, Cherry Hill, NJ 08002: On or about June 19, 2013, employees were exposed to flying wood chips while operating chain saws without wearing eye and face protection. LUMBERJACK TREE PROS, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, ITS EQUIVALENT, OR ESSENTIALLY SIMILAR STANDARD, 1910.133(a)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 315826735, CITATION 1, ITEM 2(D), ISSUED ON 9/30/11, FINAL ORDER DATE 10/19/12. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 cfr 1903.19.
