29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a. Access to two circular saws was not prevented, such as by a barrier chain or similar awareness device.  Abatement documentation is required for this violation.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a. Where maintenance work was done on saws and other equipment, such as sharpening and changing blades, a lockout program was not established and implemented, including applying a lock during maintenance, training employees on procedures and documenting the training, reviewing the program annually, and putting the program in writing.  Abatement documentation is required for this violation.

29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:  a. Where sound level meter reading indicated noise exposure up to 92.5 dBA, a hearing conservation program had not been established including annual hearing tests.  (Note: this violation can be abated by providing annual hearing tests without first conducting a monitoring study).  Abatement documentation is required for this violation.
