29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could not walk directly into the opening:    a)    The catwalk in the bin area had two openings for vertical ladders which had to be stepped across for platform access which were approximately 31 inches x 15 inches and 30 inches x 21 inches respectively.    b)    The elevator pit and pit access ladder opening was not guarded.  Only one chain was provided to protect access to the pit opening, and that chain sagged to 29 inches above the workfloor in the middle.    ABATEMENT CERTIFICATION REQUIRED
