29 CFR 1910.147(d): 29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) covered the following elements, however, the actions were not done in sequence as required by 29 CFR 1910.147 (d)(1)-(6): On or about 7/8/2013 employees did not implement lock out/tag out procedures when accessing the floor areas inside the 17 ROTO Mold Machines to clean out parts dropped into the machine operation area during production.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. On and before 7/8/2013, the employer did not ensure ROTO Mold Machine #2 and ROTO Mold Machine #4 was adequately guarded to protect operators from pinch points created between the rotating mold and the operator's platform guardrail as the mold cycles into the oven. b. On and before 7/8/2013, the employer did not ensure ROTO Mold Machine #2 was adequately guarded to protect employees from pinch points created between the rotation of the mold between the oven and cooling chambers at the rear area of the machine. An electric fan and squeegee were placed inside the guardrails used to protect employees from the rotating molds.
