29 CFR 1910.132(d)(1):  The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):   A personal protective equipment assessment was not performed for each job position to determine the types of personal protective equipment needed for employee(s) changing propane tanks on powered industrial trucks, employee(s) lifting heavy materials, fall protection for order fillers using powered industrial trucks to access products in the upper storage racks, and etc., thus exposing all employee(s) to struck-by hazards.
