29 CFR 1910.132(b): Employee-owned equipment. Where employees provide their own protective equipment, the employer shall be responsible to assure its adequacy, including proper maintenance, and sanitation of such equipment.     a. Welding Department/Row B - booth # 10 - On or about July 11, 2013, the employer did not ensure  proper maintenance of employee's owned personal protective equipment. Employees (welders) were exposed to potential physical hazards. The conditions of the personal protective equipment were the following;      1. The interior components of a welding helmet - XTreme Garage - were being held in place at two locations by masking tape ( battery package component and filter shade adjustment button).       2. The (felt clothing material) head adjustment pieces inside the welding helmet - XTreme Garage -  were being held together with two metal staples.

29 CFR 1910.252(c)(1)(ii): Screens. When welding must be performed in a space entirely screened on all sides, the screens shall be so arranged that no serious restriction of ventilation exists. It is desirable to have the screens so mounted that they are about 2 feet (0.61 m) above the floor unless the work is performed at so low a level that the screen must be extended nearer to the floor to protect nearby workers from the glare of welding.    a. Production/Welding Department/Rows A through E - On or about July 11, 2013, employees were required to walk along the ailes/rows where welder's booths had welding curtains showing burns, marks, and holes. Employees were thereby exposed to physical hazards. The locations of welding curtains showing burns, marks, and holes were such as but not limited to the following;    1. The front welding curtain in row B at booth # 8.    2. The front welding curtain in row B at booth # 10.    3. The front welding curtain in row C at booth # 5.    4. The front welding curtain in row E at booth # 1.
