29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:     A)    The United States Postal Service, located at 200 North Diamond St., Mansfield, OH 44901: On or about July 9, 2013, the employer did not ensure that performance of the employee, operating powered industrial vehicle Big Joe (Model PDCM-20-60, Serial # 351410), was evaluated at least once every three years, exposing employees to struck-by, caught-in-between, and tip-over hazards in a case of an unqualified operator not operating the forklift in a safe manner.    The United States Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1910.178(l)(4)(iii)), which was contained in OSHA Inspection # 311517528, Citation number 2, Item number 1, and was affirmed as a final order on 12/07/2010, with respect to a workplace located at 1165 2nd Avenue, Des Moines, IA 50318.    No additional abatement information is needed for this item.
