29 CFR 1910.305(b)(2)(i): Employer did not ensure that covers of outlet boxes having holes through which flexible cord pendants pass were provided with bushings designed for the purpose or had smooth, well-rounded surfaces on which the cords may bear: a) On or about July 9th 2013, and at times prior, employer did not ensure that electrical outlet boxes having holes through which the flexible cords pass, were provided with bushings or smooth, well rounded surfaces to prevent damage to the cord. A switch box powering an infrared light cabinet in the shop area, showed missing bushings and wire insulation damage. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.305(g)(2)(iii): Employer did not ensure that flexible cords and cables were connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: a) On or about July 9th 2013, and at times prior, employer did not ensure that strain relief was provided to cords going into the switch case of an infrared cabinet in the shop area. b) On or about July 9th 2013, and at times prior, employer did not ensure that strain relief was provided to the power cord of a Dayton wet/dry vacuum model 1UG91B used in the main shop area. c) On or about July 9th 2013, and at times prior, employer did not ensure that strain relief was provided to an electrical plug providing power to a fluorescent light fixture in the center bay of the main shop. 29 CFR 1910.157(e)(3): The employer did not ensure that portable fire extinguishers were subjected to an annual maintenance check: a) On or about July 9th, 2013, and at times prior, Safelite fullfillment Inc. did not ensure that their fire extinguishers were subjected to the required annual maintenance check. At least five fire extinguishers were observed mounted on walls in the warehouse area. All of them with expired maintenance check tags. Abatement Note: Abatement certification is not required for this item.
