29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form or scrap lumber with protruding nails was not kept clear from work areas. On or about July 9, 2013 employees of LHG Construction were exposed to slip, trip, fall, struck-by, and impaling hazards while working on the clubhouse due to an area that was littered with building materials including board with protruding nails.

29 CFR 1926.300(a): Hand tool(s), power tool(s) or similar equipment were not maintained in a safe condition. On or about July 9, 2013 employees of LHG Construction were exposed to electrical hazards while working with a circular saw that had the electrical cord restraint pulled loose exposing employees to live electrical current.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about July 9, 2013 employees of LHG Construction were exposed to fall hazards in excess of six feet while working on the clubhouse roof.
