29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: Employees performing work in an excavation greater than 6 feet deep were not trained in the recognition of hazardous conditions. At this job site the followiing examples of lack of training were observed: A. Not all employees performing work in the trench were aware of at what depth of a trench or excavation a protective system was required. B. The competent person was not able to identify the classification of soil being excavated.

29 CFR 1926.652(b): The slopes and configurations of sloping and benching systems were not in accordance with the requirement options of 29 CFR 1926.652(b)(1), (b)(2), (b)(3), or (b)(4): At this sanitary sewer installation project, employees worked in a trench corner where the benching system left a vertical wall greater than 5 feet in height. Employees were exposed to hazards associated with a trench collapse.
