29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: The employer does not develop, document and utilize procedures for the control of potentially hazardous energy. a. This violation occurred on or about July 9, 2013, in Plant 1 where employees were exposed to a struck-by and electrical shock hazard when servicing a conveyor system without written procedures for the control of potentially hazardous 220VAC and pneumatic energy. b. This violation occurred on or about July 9, 2013, in Plant 2 where employees were exposed to a struck-by and electrical shock hazard when servicing a conveyor system without written procedures for the control of potentially hazardous 220VAC and pneumatic energy. c. This violation occurred on or about July 9, 2013, in Plant 3 where employees were exposed to a struck-by and electrical shock hazard when servicing a conveyor system without written procedures for the control of potentially hazardous 220VAC and pneumatic energy. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that procedures are developed, documented and utilized for the release of potentially hazardous energy.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one half hour: The employer does not separate oxygen cylinders in storage from fuel-gas cylinders. This violation was observed on or about July 9, 2013, on the north exterior of Plant 2 where employees were exposed to a fire hazard when oxygen and acetylene compressed gas cylinders were stored without being separated. The cylinders were approximately 1 foot apart. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that oxygen cylinders in storage are separated from fuel-gas cylinders a minimum of 20 feet or by a noncombustible barrier at least 5 feet having a fire-resistance rating of at least one half hour.
