29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:     a)  Adarand Constructors, Inc. at the Bridge on Interstate 70 near Wolcott, CO 81655:  On or before July 3, 2013, the employer did not ensure employees were protected from falling when core drilling and installing bolts for a new guardrail system at the edge of the 1200 foot long bridge at Mile Marker 154.  Employees used 6 foot shock absorbing lanyards without a horizontal lifeline system, and these lanyards did not provide 100% fall protection for employees walking and working near the edge of this bridge.  Condition exposed employees to fall hazards of up to 30 feet.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds per attached employee, nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two, and under the supervision of a qualified person:  a)  Adarand Constructors, Inc. at the Bridge on Interstate 70 near Wolcott, CO 81655:  On or before July 3, 2013, the employer did not ensure that the personal fall arrest system anchorage points maintained a safety factor of at least two, and did not ensure the anchorages were installed by a qualified person in the following instances:    1)  employees connected to two retractable lanyards that they anchored to handles on the back of the trailer, and the handles were not tested or approved as 5,000 pound anchorage points by a qualified person.  The handles were 1/4 inch in diameter, and were welded to the trailer shell; and  2)  employees connected to two retractable lanyards that they anchored to an open hook in the front bumper of the truck, and the open hook did not qualify as an approved anchorage point.  Conditions exposed employees to fall hazards of up to 30 feet.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet nor contact any lower level:  a)  Adarand Constructors, Inc. at the Bridge on Interstate 70 near Wolcott, CO 81655:  On or before July 3, 2013, the employer did not ensure personal fall arrest systems were rigged to prevent a fall of more than 6 feet when connecting a self-retracting lifeline to the hook on the front bumper of the truck at the 1200 foot long bridge at Mile Marker 154.  Condition exposed employees to fall hazards of up to 30 feet.
