29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  For employee(s) who performed servicing or maintenance on equipment including but not limited to the TOYO plastic injection molding machines, some with robots, a lockout tag out written program had not been developed including but not limited to a written program, machine-specific procedures, and periodic inspections.

29 CFR 1910.212(a)(1) One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Main Production floor, Polymer Machinery Corp "Auger Grinder" Model V835, used with TOYO Plastar Si-55 III Plastic Injection Molding Machine #12 on the day of the inspection, had an unguarded auger.    b) Main Production floor, Polymer Machinery Corp "Auger Grinder" Model V835, used with TOYO Plastar Si-55 III Plastic Injection Molding Machine #12 on the day of the inspection, had an unguarded rotating bearing shaft(s).  c) Main Production floor, Polymer Machinery Corp "Auger Grinder" SN 68A67, used with TOYO Plastar Si-55G Plastic Injection Molding Machine #11 on the day of the inspection, had an auger which was not guarded in the front.  d) Main Production floor, Polymer Machinery Corp "Auger Grinder" SN 68A67, used with TOYO Plastar Si-55G Plastic Injection Molding Machine #11 on the day of the inspection, had an unguarded rotating bearing shaft(s).    e) Molding Clean Room, Nelmor "Auger Grinder" Model AN1010M, SN 851123616, used with TOYO Plastar Ti-110H2 Plastic Injection Molding Machine #14 on the day of the inspection, had an unguarded rotating bearing shaft(s).  f) Warehouse, Johansson drill press (SN 339641) had an unguarded chuck.

29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel:  Main Production floor, by the TOYO Plastic Injection Molding Machine #3,  the Dayton pedestal grinder did not have a tongue guard which could adjust to the constantly decreasing diameter of the abrasive wheel.  The opening between the grinder and the abrasive wheel measured approximately 1 inch.
