29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.      Location: 8711 18th Ave., Brooklyn, NY.     a) Employer did not ensure that the tubular welded scaffold utilized by employees was provided with means of access; on or about 7/10/13.

Location: 8711 18th Ave., Brooklyn, NY.     a) Employer did not ensure that the tubular welded scaffold utilized by employees was fully planked. Employees were exposed to a potential fall of up to approximately 13 ft to the floor below; on or about 7/10/13.    Adventure Masonry Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(b)(1), which was contained in OSHA inspection number 312720972, citation number 1, item number 1 and was affirmed as a final order on 8/14/09.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.       Location: 8711 18th Ave., Brooklyn, NY.     a) An employee was exposed to a potential fall of approximately 13 ft to the ground below while working from a tubular welded scaffold without fall protection; on or about 7/10/13.
