29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws.  On or about July 9, 2013, west side of the residential home, employees using an extension cord with the outer sheathing pulled away from the plug end exposing the internal wiring were exposed to the hazard of electrical shock while operating a DeWalt skill saw.
