29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On or about July 9, 2013, on the east side of the residential construction site, employees who worked without fall protection were exposed to a fall hazard of approximately 10 feet while engaged in framing activities.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided. On or about July 9, 2013, on the east side of the construction site, employees without a ladder used wall cross bracing and studs to climb up and down the one-story building exposing the employees to a fall of more than 10 feet.
