29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows: (a) North Side, Third Floor - On or about July 9, 2013 employees were exposed to a fall hazard while working on a scaffold that was not fully decked or planked. There was a 38 inch gap between scaffold decks.

29 CFR 1926.451(e)(8): Direct access to or from another surface shall be used only when the scaffold is not more than 14 inches (36 cm) horizontally and not more than 24 inches (61 cm) vertically from the other surface. (a) North Side, Third Floor - On or about July 9, 2013 employees were exposed to a fall hazard due to the scaffold direct access from the scaffold walkway to a steel beam was measured at 49 inches.

29 CFR 1926.454(b): The employer shall have each employee who is involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question. The training shall include the following topics, as applicable: (a) Jobsite - On or about June 9, 2013 employees were exposed to a fall hazard while working on scaffolds without training on how to assemble, disassemble, operate and inspect scaffolds.
