29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft. The employer does not guard projecting shaft ends. This violation was observed on or about July 10, 2013, on the south dryer iin the X-ray film building. PETAG CORPORATION, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.219(c)(4)(i) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 315675421, CITATION NUMBER 01, ITEM 004 AND WAS AFFIRMED AS A FINAL ORDER ON MARCH 22, 2013, WITH RESPECT TO A WORKPLACE LOCATED AT 2103 LYONS AVE., HOUSTON, TX 77020. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that guards are installed on all projecting shaft ends.
