29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: (a). Worksite located at 401 N. 8th Street, Savannah, Missouri- Employees were exposed to a shock or electrocution hazard in that the employer had not inspected the extension cord being used on the job location. The cord had its' outer insulation breached as well as it was covered in tar, making it impossible to inspect properly for empoyee safety.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: (a). Worksite located at 401 N. 8th Street, Savannah, Missouri- Employees were exposed to fall hazards while working on a flat roof and were not utilizing any means of fall protection. All three employees located on the roof were engaged in roofing work and were not being monitored by the use of a safety monitor.
