Worksite located at 401 N. 8th Street, Savannah, Missouri- Employees were exposed to a shock or electrocution hazard in that the employer had not inspected the extension cord being used on the job location. The cord had its' outer insulation breached as well as it was covered in tar, making it impossible to inspect properly for empoyee safety.

Worksite located at 401 N. 8th Street, Savannah, Missouri- Employees were exposed to fall hazards while working on a flat roof and were not utilizing any means of fall protection. All three employees located on the roof were engaged in roofing work and were not being monitored by the use of a safety monitor.
