29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification On or about July 10, 2013 at the above addressed jobsite, the employer did not ensure that the temporary electrical wiring on the jobsite was free from recognized hazards. Employees were using portable power taps and were exposed to electrical hazards. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level: On or about July 10, 2013 at the above addressed jobsite employees were performing siding work from a pump jack scaffold and were not protected by end or mid rails. Employees were exposed to a fall of approximately 20.9 feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
