29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent: (a) Deerfoot Timber Sale, Hayden, Idaho; on or about July 9, 2013, and at times prior thereto, landing man/yarder operator employee(s) were exposed to eight-hour time-weighted averages of 89-91 decibels (dBA), with peak levels of approximately 146 dBA. (b) Deerfoot Timber Sale, Hayden, Idaho; on or about July 9, 2013, and at times prior thereto, sawyer employee(s) were exposed to eight-hour time-weighted averages of 92-93 decibels (dBA), with peak levels of approximately 137dBA. (b) Deerfoot Timber Sale, Hayden, Idaho; on or about July 9, 2013, and at times prior thereto, sawyer employee(s) were exposed to eight-hour time-weighted averages of 92-93 decibels (dBA), with peak levels of approximately 137dBA. (a) Deerfoot Timber Sale, Hayden, Idaho; on or about July 9, 2013, and at times prior thereto, Yarder first aid kit did not have one package gauze roller bandage at least 2" wide; two triangular bandages; and two large 8x10 bandages (only one was present). (b) Deerfoot Timber Sale, Hayden, Idaho; on or about July 9, 2013, and at times prior thereto, Dodge Pickup first aid kit did not have one package gauze roller bandage at least 2" wide; adhesive tape; and latex gloves. (c) Deerfoot Timber Sale, Hayden, Idaho; on or about July 9, 2013, and at times prior thereto, Sawyer pickup first aid kit was missing from the vehicle. Note: Abatement certification is required for this item.
