The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated. On or about July 11, 2013 at the worksite; an employee was observed using the 340AJ JLG aerial lift to access and work on a 19 ft. high roof without wearing any type of fall protection harness, body belt, or lanyard.

On or about July 11, 2013 at the worksite; employees were working on a 19 ft. high 4/12 pitch roof with no conventional fall protection measures being used on the open edge of the office roof.

On or about July 11, 2013 at the worksite; an employees was working on a 17.3 ft. high 6/12 pitch roof with no conventional fall protection measures being used on the open edge of the roof eave. The employee was installing valley flashing and exposing him to an area that the safety nets did not cover.
