Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.    a.  At the workplace, located at 2191 Westbelt Drive, Columbus, Ohio, on the West wall of the warehouse, an electrical junction box with live wires did not have a cover installed, exposing employees to an electrical hazard.    b.  At the workplace, located at 2191 Westbelt Drive, Columbus, Ohio, on the South wall of the main enterance, an electrical outlet box with live wires did not have a faceplate installed, exposing employees to an electrical hazard.
