29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent):    a) Tembec BTSLR, Inc. - Toledo, Ohio: On or about July 24, 2013, employees were exposed to fall hazards of approximately 62 inches when accessing the platform located on the C-Dryer.  The platform was not equipped with a standard guardrail that included a mid-rail.

29 CFR 1910.146(f)(4): The entry permit that documented compliance and authorized entry to a permit space did not identify the authorized entrants within the permit space by name or by such other means as will enable the attendant to determine quickly and accurately, for the duration of the permit, which authorized entrants are inside the permit space:    a) Tembec BTSLR, Inc. - Toledo, Ohio: On or about July 9, 2013, the employer did not identify authorized entrants on the entry permit associated with entry into the C-Dryer.  A permit which does not indicate authorized entrants of a permit required confined space could hinder prompt identification of authorized entrants by the attendant and also the implementation of rescue operations in the event of an emergency.  Employees who entered the dryer for cleaning purposes were exposed to potential atmospheric and mechanical hazards while in the permit required confined space.    b) Tembec BTSLR, Inc. - Toledo, Ohio: On or about July 10, 2013, the employer did not identify authorized entrants on the entry permit associated with entry into the C-Dryer.  A permit which does not indicate authorized entrants of a permit required confined space could hinder prompt identification of authorized entrants by the attendant and also the implementation of rescue operations in the event of an emergency. The employer did not assure a level of safety equivalent to that obtained by using a lockout system was achieved by implementing additional safety measures while employees cleaned the dryer.
