29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in detail as required by the regulation:  a) On or about July 9, 2013, at the above located workplace, the employer's OSHA 300 form for 2013 was  not completed in detail as required by 1904.4 through 1904.12. In that:  i) The 2013, OSHA 300, included approximately 15 injuries (that occurred between March 1, 2013 and July 9, 2013), that did not include the number of loss work days as required by the standards.  ii) The 2013, OSHA 300, included approximately 4 sharps injuries (that occurred between March 1, 2013 and July 9, 2013), that listed the employees name in col. B.

29 CFR 1910.1030(c)(1)(iv):   The Exposure Control Plan was not reviewed and updated at least annually and whenever necessary to reflect new or modified task and procedures which affect occupational exposure and reflect new or revised employee positions with occupational exposure. The review and update of such plan did not:  i) Reflect changes in technology that eliminate or reduce exposure to bloodborne pathogens; and  ii) Document annually considerations and implementations of appropriate commercially available and effective safer medical devices designed to eliminate or minimize   occupational exposure.    a) On or about July 9, 2013, at the above located workplace, where employees were exposed to blood and other potentially infectious material, the workplace bloodborne pathogen exposure control program had not been reviewed with written documentation since December 2009.

29 CFR 1910.1030(c)(1)(v): The employer, who was required to establish an Exposure Control Plan did not solicit input from non-managerial employees responsible for direct patient care who are potentially exposed to injuries from contaminated sharps in the identification, evaluation, and selection of effective engineering and work practice controls and did not document the solicitation in the Exposure Control Plan:    a) On or about July 9, 2013, at the above located workplace, employees experienced sharps injuries since March 1, 2013. The employer did not solicit input from non-managerial employees responsible for direct patient care in the identification, evaluation, and selection of effective engineering and work practice controls and had not documented the solicitation.

29 CFR 1910.1030(f)(3)(i): The postexposure evaluation and followup of an exposure incident did not include documentation of the route(s) of exposure or the circumstances under which the exposure incident occurred:      a) On or about July 18, 2013, at the above located workplace an employees experienced and reported an exposure incidents.   The employer's post exposure investigation did not include sufficient documentation concerning the circumstances surrounding the exposure incident. The investigation did not include:     a) Engineering controls in use at the time,   b) Work practices followed,  c) A description of the device in use  d) Protective equipment or clothing that was used at the time of the exposure incident  e) Location and procedure being performed when the incident occurred,   f) Description of the device in use (including brand and type), and  g) The employee's training
