East Coast Steel Fabrication, Inc. Rockbridge Dredge: On or about 07/08/13, a guardrail was not erected around a large opened stairway creating a fall hazard of 10 feet.

29 CFR 1915.73(d): When employees are exposed to unguarded edges of decks, platforms, flats, and similar flat surfaces, more than 5 feet above a solid surface, the edges shall be guarded by adequate guardrails meeting the requirements of 1915.71(j)(1) and (2), unless the nature of the work in progress or the physical conditions prohibit the use or installation of such guardrails. a.) Rockbridge dredge: On or about 07/08/13, employee were not provided fall protection while working on top of the deckhouse/superstructure in that a guardrail system was not erected along the flat edge thereby exposed employees to an 11' fall.

The employer must develop and implement a written fire safety plan that covers all the actions that employers and employees must take to ensure employee safety in the event of a fire. (See Appendix A to this subpart for a Model Fire Safety Plan.) a.) Rockbridge Dredge: On or about 07/08/13, a written fire safety plan was not developed and implemented for employees conducting hot work operations. Note: The employer must include the following information in the fire safety plan: Identification of the significant fire hazards; Procedures for recognizing and reporting unsafe conditions; Alarm procedures; Procedures for notifying employees of a fire emergency; Procedures for notifying fire response organizations of a fire emergency; Procedures for evacuation; Procedures to account for all employees after an evacuation; and Names, job titles, or departments for individuals who can be contacted for further information about the plan.
