29 CFR 1910.305(d)(2): Panelboard enclosures. Panelboards shall be mounted in cabinets, cutout boxes, or enclosures designed for the purpose and shall be dead front. However, panelboards other than the dead front externally-operable type are permitted where accessible only to qualified persons: On or about 7/9/2013, ithe employer allowed employee(s) to be exposed to the 440 volt electrical panel box that was missing the dead front panel located in the electrical room.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: On or about 7/9/2013, the employer allowed employeesd to walk in close proximity to the energized 440 volt electrical lines located on the rear wall of the warehouse. The 440 volt energized electrical lines had been removed from the junction box.
