On or about July 10, 2013, the employer allowed employees to use extension cords as permanent wiring a) The extension cord ran from the outlet to a small work light that was attached to a Wilton Twenty milling machine. b) A red extension cord that had damage to the outer casing ran from an outlet to an outlet extender where a radio and another extension cord were plugged into the outlet extender; the second extension cord was used as permanent wiring it was attached the workbench with zip ties. The extension cord was damaged due to over current.
