29 CFR 1926.404(b)(1)(ii): Ground-fault circuit interrupters. All 120-volt, single-phase 15- and 20-ampere receptacle outlets on construction sites, which are not a part of the permanent wiring of the building or structure and which are in use by employees, shall have approved ground-fault circuit interrupters for personnel protection. Receptacles on a two-wire, single-phase portable or vehicle-mounted generator rated not more than 5kW, where the circuit conductors of the generator are insulated from the generator frame and all other grounded surfaces, need not be protected with ground-fault circuit interrupters. Piscataquis River Bridge - The Powermate generator, supplying power to the Oztec vibrator and extension cord, was not equipped with a GFCI.

29 CFR 1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided. In the following instances employees accessed and/or egressed breaks in elevation of more than 19 inches and the embankments had slopes greater than 25 degrees: a) Piscataquis River Bridge - Employees climbed the northeast embankment. b) Piscataquis River Bridge - Employees climbed the southwest embankment. T-Buck Construction, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 313575284, citation number 2, item number 1 and was affirmed as a final order on 9/10/2009, with respect to a workplace located at Hammond Street Bridge, Bangor, Maine.
