29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part:    Jobsite: Site.    a) The company had no elements of a written Lockout/Tagout (LOTO) program and had not adequately conveyed proper LOTO procedures to the employees.    b) The company did not adequately convey safe electrical work practices and procedures to protect its employees from arc flash/blast or electrocution while working on known energized circuits or for voltage verification.

All employees had not been instructed in the recognition of known hazards and the regulations applicable to safe electrical work practices to prevent or control the hazards where:    a) the company had no lockout/tagout (LOTO) program.    b) Neither the company owners nor the employees were aware of or utilizing proper safe electrical work practices even though the company had a copy of the 2009 NFPA 70E Safe Electrical Work Practices handbook.    c) Employees had not been adequately trained in the use of personal protective equipment (PPE).

An employee was shocked and burned after contacting a known energized 277 volt electrical circuit.
