29 CFR 1904.29(b)(1): What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year. a. Case 2013-00027, on February 07, 2013, in the OPC (outpatient clinic) suffered a needle stick to left thumb during injection. The log and the incident report failed to clearly identify where the injury occurred as required under section E of the OSHA 300 form. b. Case 2013-00032, on March 01, 2013, in the OPC suffered a needle stick to the left hand. The log and the incident report failed to clearly identify where the injury occurred as required under section E of the OSHA 300 form. Additionally the column F states that it was a left hand stick, however the incident report states the employee was stuck in the left thumb. c. Case 2013-00038, on June 06, 2013, in the Other (non-patient care area) suffered a sharps exposure. The log and the incident report failed to clearly identify where the injury occurred as required under section E of the OSHA 300 form. The injury occurred in the Geriatrics Extended Care unit according to the incident report. Additionally the column F does not clearly state what the effected body part was. The incident report states that injury occurred on the right hand and punctured the 3rd digit.
