On or about July 11, 2013, on the west side of the building, the ladder used to access the roof did not extend 3 feet above the landing, exposing an employee to a 16 feet fall hazard.

The J & D Welding, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1926.760(a)(1) which was contained in OSHA inspection number 311817837 citation number 01, item number 001, issued on October, 20, 2008, with a final order date of November 17, 2008, with respect to a workplace located at 551 N. Nova Rd., Daytona Beach, FL. 29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: a. On or about July 11, 2013, on the edge of a flat roof, employees engaged in steel erection activities were not protected from a 16 feet fall hazard by the use of a fall protection system.
