Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) Adam & Associates on or about July 15, 2013, at the address of 1800 Cleveland Ave. Sandusky, OH: The employer failed to ensure employees were protected from falls while performing roofing work on a low sloped commercial type structure. Employees working in this condition were exposed to falls of approximately 10 ft. to the ground below.
